Reset to Rise supports individuals and families during difficult transitions — including people returning home from incarceration, individuals discharged from hospitals or mental health facilities, people experiencing homelessness, and children struggling in school.
We provide case management, connect individuals to mental health and medical care, assist with Medicaid and food assistance, support school advocacy for children, and offer access to food and clothing when needed.
Our goal is simple: to provide stability, guidance, and real support so people can reset and rise.

Our program helps children get the support they need early — before small challenges turn into bigger problems.
Too often, children with emotional or behavioral struggles are labeled or punished instead of helped. We work to change that.
We provide mental health services and help families work with schools so children can stay on track and succeed.
Licensed therapists and nurse practitioners guide all services, making sure children receive proper care while parents get support navigating the school system.

The Heart House of Palmer Alliance Food Pantry Program is a compassionate, community-driven initiative dedicated to reducing food insecurity and easing the stress placed on individuals and families facing hardship. Operated by people who genuinely care, the program provides monthly food assistance to clients while fostering dignity, encouragement, and hope. The pantry is made possible through strong community partnerships and generous donations that reflect a shared commitment to wellness and stability. In addition to providing nutritious food and essential staples, the program works alongside Behavioral Health Professionals who understand the connection between basic needs and mental health, ensuring clients receive holistic support.
